Log into My HealtheVet (Premium) and pull a Blue Button report: select VA Notes for the last 12–24 months and include Telephone/Triage/Telehealth entries. Download the PDF, highlight lines showing severity, activity limits, ER referrals, or med changes. On VA.gov, open your claim and upload it as additional evidence with clear filenames (e.g., “MigraineTriage2025-03-14”).
