Draft a one-page "Request for Amendment of Health Information" listing: note date/provider, the exact wrong statement, and your corrected statement with supporting proof (labs, messages, logs). Send it today to your VA facility Privacy Officer/Release of Information (find contact on VA.gov). Then upload your request and evidence to your claim so the rater sees the correction in progress.
