DBQ Power Move: Download Your Condition’s DBQ and Get It Completed Today

DBQs win claims—put one in your file today. Go to VA.gov, find the Disability Benefits Questionnaire for your primary condition, download it, and send it to your treating provider (secure message, email, or fax) asking them to complete, sign, and date it based on your current exam. Upload the finished (even partial) DBQ to your claim under Supporting Documents today. If your provider won’t fill it out, ask them to write a note addressing the same DBQ items and upload that.
