Templates FAQ
What evidence do I need before filing a VA disability claim?
At minimum, you need three things: (1) evidence of an in-service event, injury, or exposure; (2) a current medical diagnosis; and (3) a nexus—a link connecting the two. A doctor's opinion stating your current condition is "at least as likely as not" related to service is the gold standard.
Our evidence checklist templates help you organize your packet before filing. For a deeper dive, VA Claims Explained covers what raters look for and how to build a solid case.
Can I customize and edit the templates?
Yes—download the file, make a copy in your preferred editor (Word, Google Docs, etc.), and adjust it for your specific claim. Keep the section headings intact so VA raters can easily follow along.
All templates are available on the Templates page.
Will these templates work for any VA claim?
Most templates are diagnosis-agnostic—they work for any condition because they focus on the structure and format VA expects. Pair them with the relevant playbook or Field Manual for condition-specific language and evidence examples.
How often do you release new templates?
New templates publish whenever we identify a gap or when VA policy changes require updated formats. Sign up for email updates to get notified when new downloads are available.
