Go to VA.gov, download VA Form 21-8940, and fill it tight: exact last day worked, hours/week, and earnings for each job in the past 5 years; list only service-connected conditions that stop you working; include education/training and treatment sources. Upload the signed form to your active claim (or start a new one for IU) and attach recent pay stubs/W‑2s; ask your last employer to complete 21-4192 to prevent delays.
